Caffeine and anxiety: how much is too much

Caffeine can cause anxiety, and it does so in people who have no anxiety disorder at all. The effect grows with the dose and is clearest above 400 mg a day, about four mugs of filter coffee.

In people with panic disorder it goes further. A dose of around 480 mg set off a panic attack in half of patients in controlled studies.

Does caffeine cause anxiety?

A 2024 analysis pooled 14 studies of healthy adults. Caffeine raised anxiety overall. At lower doses the effect was moderate. Above 400 mg it was several times stronger.

Two things follow. The effect is not imagined, and it is not limited to people who are already anxious. And dose is the lever: the same person can be fine on one cup and on edge after three.

Why caffeine feels like anxiety

Caffeine blocks adenosine, the signal that slows the brain down, and it pushes the body toward a state of alert. The heart beats faster, the hands can shake, the stomach tightens, and breathing becomes shallower.

Those are the same sensations as anxiety. If you are prone to worry, the body's signals arrive first and the mind supplies a reason afterwards.

A fast heart after two coffees is caffeine. The thought that something must be wrong is the part that can be argued with.

How much caffeine is too much for anxiety?

There is no threshold that applies to everyone. These are the figures worth knowing:

  • 400 mg a day is the general limit for healthy adults, and the level above which the anxiety effect was strongest.
  • 200 mg at once is the most the European Food Safety Authority considers safe as a single dose. For anxiety, the single dose matters more than the daily total, because the peak is what you feel.
  • One strong drink can be enough if you are sensitive. A can of Celsius holds 199 mg and a large Cold brew about 180 mg.

Can caffeine cause panic attacks?

In people with panic disorder, yes, and at a dose many people reach in a day.

A 2022 review gathered nine placebo-controlled studies, covering 237 patients. After caffeine, 51 percent had a panic attack. After a placebo, none did. In the six studies that included healthy volunteers, under 2 percent of them had one.

The dose was almost always 480 mg, roughly five cups of coffee taken together. Much less is known about smaller amounts, and the authors say so. What the studies show is that people with panic disorder react to caffeine in a way that healthy people do not.

If you have panic disorder, that is a reason to talk to your doctor about caffeine. It is not a reason to assume every cup is dangerous.

How to cut caffeine anxiety without quitting

Halve the dose, do not remove it. Stopping at once brings a withdrawal headache and low mood for up to nine days, which is a poor trade for someone who is already anxious.

Spread it out. Two small coffees three hours apart peak lower than one large one. The peak is what produces the symptoms.

Eat first. On an empty stomach caffeine is absorbed faster and peaks higher.

Switch the drink. A cup of Black tea has about half the caffeine of a mug of coffee, and Green tea less than a third. The comparison of tea and coffee has the numbers.

Keep it to the morning. Caffeine late in the day costs sleep, and a short night makes the next day more anxious whatever you drink. The cutoff for your bedtime is the hour to aim for.

Count the sources you forget. Pre-workout, cola, energy drinks and some painkillers all add to the same total.

When to see a doctor

Caffeine can make anxiety worse. It is rarely the whole cause. See a doctor if the anxiety continues after two weeks on much less caffeine, if you have panic attacks, or if a racing or irregular heartbeat comes on without caffeine.
